Programa Pacient Expert Catalunya: una estratègia per potenciar l’autorresponsabilitat del pacient i el foment de l’autocura
Abstract
The Patient Expert Catalunya Program aims to enhance the self-care, co-responsibility and autonomy of people with chronic illnesses. It is a multidisciplinary initiative based on the collaboration between patients and health professionals and team work.
In the Patient Expert Catalunya Program, the protagonist is the expert patient, who transmits knowledge and shares experiences with other people who suffer from the same health problem. During the sessions, health professionals will play the role of observers who only intervene if necessary. The expert patient is the person affected by a chronic disease that is capable of taking responsibility for the disease itself and taking care to identify the symptoms, responding to them and acquiring tools to manage the physical, emotional and social impact of the illness.
