dc.description.abstractDuring the period of validity of the Health Plan of Catalonia 2011-2015, annual conferences were organized in which the professionals of the system could present, in the form of a poster, their work experiences related to the Health Plan projects. The participation increased year after year, up to more than three thousand experiences presented at the end of the period. The main lines of action and thematic are analyzed and the new plan is valued.
