Abstract
The aging of the population and the increase in life expectancy implies an increase in the population of advanced age and, therefore, of individuals with chronic health problems and high comorbidity. The complexity of care for people at risk is a challenge for professionals, managers and service providers. Continuous readmission is a significant increase in the cost of healthcare. A large number of interventions are described to reduce the risk of unplanned hospital re-entry in risk populations, or in a so-called fragile population.
