Primary shoulder arthroplasty; Spanish Health System; Clinical effectiveness
There are two types of joint replacements within the primary shoulder arthroplasty procedure, depending on which parts are replaced by a prosthesis. A partial arthroplasty replaces only the portion of the joint corresponding to the upper arm (proximal humeral head), while a total arthroplasty replaces said portion as well as the glenoid cavity and scapula, which corresponds to the back. In this branch of surgery, one of the most outstanding technological advancements has been the incorporation of inverted models, which change the role between the prosthetic component of the proximal humerus and glenoid cavity of the scapula. In the Spanish Health System, approximately 25,000 shoulder replacements and 2,500 shoulder replacement revisions were performed between 2000 and 2015. This type of intervention was primarily indicated for patients who had pain and functional limitation resulting from trauma, degenerative disease, or massive, chronic rupture of the shoulder joint tendons. The objective in all cases was to achieve a useful, stable and pain-free joint. Given the growing volume of this type of procedure in recent years, and the technological and training requirements for its execution, this report has two main aims. First, to describe the results of prosthetic replacement procedures in the shoulder joint and the characteristics of patients intervened in the Spanish Health System and, second, to describe the results of shoulder prostheses using the information available in arthroplasty registries. These two objectives strive to facilitate decision making and promote the future evaluation of these types of procedures in comparison with other therapeutic alternatives.
