Cas clínic: ús de la glucarpidasa en la intoxicació per altes dosis de metotrexat en una nena amb leucèmia limfoblàstica aguda
Abstract
In the administration of HDTMX, the most important thing is to avoid the development of serious toxicities, as well as the use of glucarpidase. To do this, it is necessary to maintain hyperhydration, force diuresis, ensure good urinary alkalinization and adequate rescues with leucovorin until plasma MTX levels are below 0.1-0.15 micromol/L. It is essential to verify correct renal function and the absence of a third space before starting the infusion of MTX, as well as avoid the concomitant use of drugs that may decrease its excretion (penicillins, NSAIDs, nephrotoxic drugs, etc.).
