Patients with adverse reactions due to hypersensitivity induced by NSAIDs represent a heterogeneous group with a very diverse clinical expression. NSAIDs are the first cause of hypersensitivity to drugs. Hypersensitivity reactions to NSAIDs are type B adverse reactions (unpredictable that happen in susceptible individuals) according to the OMS classification. Hypersensitivity to NSAIDs has been classified according to the symptoms they produce (respiratory, skin, anaphylaxis), the latency of these symptoms (immediate and delayed), the concomitant diseases that patients present (asthma, chronic spontaneous urticaria) and the possible reaction mechanism (immune with participation of IgE or predominantly of T cells and non-immune by inhibition of COX-1). There is no battery of tests to know if a patient, without ever having any reaction first, is allergic to one or several drugs. The study of NSAID allergy reactions is complex and it needs to adapt by patient and drug. In the face of a possible reaction, allergists make, in the first place, a directed anamnesis that it can provides a lot of information about the probability that a patient has an allergy to a drugs. Skin or laboratory tests help in the diagnosis, but only the controlled exposure test allows the final diagnosis. It’s important remember that there are no screening tests to know if a person is allergic or not. The study of drug allergy can confirm the diagnosis, even if the study is negative, a future sensitization is not ruled out.
