Metatarsalgia de transferencia tras cirugía del hallux valgus

Abstract
Introduction and objectives: first metatarsal osteotomy for the treatment of hallux valgus (HV) is not without complications, including transfer metatarsalgia. The term transfer metatarsalgia refers to the onset of pain at a different ray than that which is mechanically impaired. Generally, the origin of metatarsalgia is a biomechanical disorder, such as shortening of the first metatarsal, elevation of the head or metatarsal, hypo-correction of HV, or a problem of gastrocnemius tightness. The aim of this study is to analyse transfer metatarsalgia and to evaluate the relationship between shortening and elevation of the first metatarsal with its occurrence, such as the relationship between the position of the sesamoids and the onset of metatarsalgia.
Materials and methods: retrospective study of all patients undergoing surgery for HV between 2018 and 2021 at our hospital.
Results: in this retrospective study, we collected data from 454 patients who underwent HV surgery. We selected 131 patients that met the inclusion criteria, excluding those with previous metatarsalgia, surgical procedures on the lateral metatarsals, loss of follow-up, lack of weight-bearing radiographs, and other surgeries. 13.7% of the patients developed transfer metatarsalgia, on average, 9.2 months after surgery. In 84.2% of the cases of transfer metatarsalgia, conservative treatment was indicated, with the most frequent being the use of insoles. We performed osteotomies on the lateral metatarsals in the remaining cases. We formed 2 groups based on the presence or absence of metatarsalgia, and compared the means of the postoperative and preoperative differences between the length of the first and second metatarsals (M1 and M2), the elevation of the head of M1, and the sesamoids position. Statistical analysis did not reveal any significant differences between the 2 groups.
Conclusion: transfer metatarsalgia is a potential complication of HV surgery to keep in mind; our study did not find a significant relationship between shortening or elevation of the first metatarsal and the occurrence of metatarsalgia.
