Risc de dependència, abús i sobredosi per fentanil

Abstract

En els darrers 20 anys, la prescripció de fentanil i altres opioides més potents que la morfina ha augmentat, principalment amb administració transdèrmica i d'alliberament ràpid. Tot i les indicacions establertes, sovint s'utilitza en casos no autoritzats o per períodes inadequats, incrementant el risc d'efectes adversos greus, especialment quan es combina amb altres fàrmacs que afecten el sistema nerviós central (SNC), com benzodiazepines o antidepressius. Aquests factors augmenten la probabilitat de dependència, abús i sobredosi. El SEFV ha registrat poques notificacions d'aquests problemes (7,5%), tot i que les dades disponibles confirmen els riscos associats a l'ús inadequat del fentanil. Es destaca la necessitat de notificar reaccions adverses per millorar la seguretat en el seu ús.

Over the past 20 years, the prescription of fentanyl and other opioids stronger than morphine has increased, primarily through transdermal and rapid-release administration. Although their indications are well-established, fentanyl is often used in unauthorized cases or for inappropriate durations, increasing the risk of severe adverse effects, especially when combined with other drugs affecting the central nervous system (CNS), such as benzodiazepines or antidepressants. These factors heighten the likelihood of dependence, abuse, and overdose. The SEFV has recorded few reports of these issues (7.5%), although available data confirm the risks associated with inappropriate use of fentanyl. Emphasis is placed on the need to report adverse reactions to improve the safety of its use.
