Errors de medicació en les transicions assistencials
Abstract
Medication errors (MEs) are a leading cause of preventable injury and harm in healthcare systems worldwide. For this reason, the World Health Organization (WHO) dedicated its third global challenge to patient safety to the theme “Medication without harm”, highlighting healthcare transitions as one of the key areas of action and a critical point where lack of coordination and poor information transmission can lead to high-impact MEs.
